Applying for a Pharmacy License​

A Pharmacist is authorized to rent one of the pre-built Department of Health pharmacies located at the Reveille Main Hospital. If you would like to operate your Pharmacy from another location - this must be approved by the Health Secretary. You must submit your registration in these forums and await approval from the Secretary.

Requirements​

> You must have passed the Pharmacist exam and maintain a Pharmacist certification to be able to request a Pharmacy location license.
> The Pharmacy location must be within a finished plot or completed building, as defined by Department of Construction and Transport policy. You must be the legal owner or tenant of the region.
> To maintain your Pharmacy license, you must pay a $300 monthly licensing fee.

> The payment will be handled and registered by the Health Secretary.​

> The first payment will be charged within 24h of application approval; every subsequent payment is charged by the 8th day of the month.

> This means that at the start of every subsequent month, the Department will verify if you wish to maintain registration, and you will have a week to confirm or rescind your pharmacy registration, if you choose.​

> In the case of non-response by the 8th day of the month - the Pharmacy will be de-registered by default.​

> By submitting a registration, you acknowledge the fee requirement and you authorize the Department of Health to charge the amount from your in-game /balance, by using the /fine command.

Fees​

The monthly licensing fee is set at $300.
The registration fee is set at a minimum of $150, and maximum of $300.

For every day that has passed since the start of the month of registration, $5 are deducted from the maximum (until the minimum amount is reached) to calculate the registration cost.​

Application Format​

The preset format for registration is as follows:

• In-Game Username
• Discord Username (for easier coordination or communication with the Department of Health. If you do not have Discord, you can leave this empty)
• Acknowledgement
  • I have read and understood all relevant rules and policies from the Department of Health.
  • I understand that breaching Dept. Health policy may result in:
    • Eviction from a rented Dept. Health pharmacy region, or
    • the revoking of my Pharmacy license, and/or
    • the revoking of my Pharmacist license.
  • I agree to the monthly upkeep fee to maintain my Pharmacy registration.
• Pharmacy Location (i.e. c001 / cbd01) (Simply input the region name where your pharmacy is located. This may be the plot region, such as c011, or it may be a rental region within a plot, such as pharma-01.)
• Attached screenshot(s) of the Pharmacy:(attach a clear screenshot of your Pharmacy. If it is a rental region within a larger building, please showcase the entrance to the building from the exterior and then a screenshot that shows your pharmacy inside as well.)

The Department will aim to process applications within 72 hours of submission. Any notable issues or necessary changes to an application will be communicated to the applicant. You may apply >HERE<.

A list of all approved pharmacies will be kept on the forums and updated twice a month by the Department of Health. Any questions can be brought forward through the DoH Discord server, or through a government #support ticket on the main Discord server.
